SCOPE OF WORK: The purpose of the contract is to provide an architectural "facelift", modernize the building's mechanical, electrical, plumbing, security and information technology systems, and reconfigure the space allocation within the healthcare facility to better address current healthcare needs. LOCATION OF PROJECT: All work shall be performed at the Fort Duchesne PHS Indian Health Center, which is located at 6822 E. 1000 S., Fort Duchesne, Utah 84026. The health center is located in Uintah County, Utah. CONSTRUCTION DURATION: Period of Performance is 365 calendar days from issuance of the Notice to Proceed. CONSTRUCTION MAGNITUDE: The magnitude of this construction project is anticipated to be more than $10,000,000, in accordance with FAR 36.204(g).
