Description

5th Street Bridge Replacement. This project involves replacement of the existing bridge, Bridge No. 795701, which is the only access to a residential area over the Yacht Club Cut in New Smyrna Beach, Volusia County. The bridge will be replaced with a new three span concrete structure consisting of 2.5-ft shoulders, two 9-ft travel lanes, 5-ft pedestrian sidewalk, spanning approximately 160-ft in length. During the construction phase, a two lane vehicular Acrow bridge shall be installed on the south side of the existing bridge structure. Demolition entails complete removal and proper disposal of the existing bridge deck, superstructure and substructure, end bents, pilings, abutments, revetment, rip rap embankments, guardrail system, roadway approaches and in-situ soils to extend the waterway opening as shown on the plans. City Commission Award of Contract: February 28, 2023 Construction Contract Execution: March 13, 2023 Preconstruction Meeting: March 21, 2023 Notice to Proceed to Contractor: April 11, 2023 Completion of Construction: April 1, 2024 Deadline for Written Questions:November 4, 2022, 5:00pm. Each bid, if applicable, shall be accompanied by a Bid Security of not less than ten (10%) percent of the amount of the Bid Proposal. The Bid Security shall consist of a Certified Check, a Cashier's Check or a Bid Bond payable to the City of New Smyrna Beach (Owner.) In addition to scanning a copy in your response in ProcureNow, the Bid Security shall be submitted in a separate, sealed envelope addressed to the Owner, endorsed with the name of the bidder and project title and plainly marked, "Bid Security," which shall be received no later than the required Bid Opening date/time, using the label as addressed in the Instructions for Bid Submittal. Within ten (10) days after the opening of the bids, checks or bid bonds shall be returned except those deposited by the three (3) lowest bidders, and they shall be returned within seven (7) days after the execution of the Contract between the successful bidder and the Owner. The Bid Security of the successful bidder shall be returned without interest when the Contract has been approved and completely executed and the Notice to Proceed letter is issued. The Owner reserves the unqualified right, in the Owner's sole and absolute discretion, to reject any and all bids, to waive any irregularities, or to accept that bid or combination of bids, which in the Owner's sole and absolute judgment, shall, under all circumstances, best serve the public interest. Simultaneously with the delivery of the executed Contract, the successful bidder shall deliver to the Owner, if required as a part of the Contract Documents identified in "General Conditions", an executed bond in the amount of One hundred (100%) percent of the accepted bid as security for faithful performance of his Contract and for payment of all persons performing labor or furnishing materials in connection therewith, prepared on the form of bond attached hereto or the Bond Company's document, and have as surety thereon such company or companies approved by the Owner and authorized to transact business in the State of Florida. Prime Contractor must be FDOT prequalified in Work Class 17, Intermediate Bridges. The Prime Contractor or a combination of Prime Contractor and Subcontractors must be prequalified in the following additional Work Classes: 7 Drainage 10 Flexible Paving 11 Grading 15 Hot Plant-Mixed Bituminous Courses 38 Roadway Signing Bidders are advised to note the FDOT Prequalification requirements in Section 2-1, LAP Division 1 Specifications of the Bid Documents. Bidders and subcontractors must provide documentation substantiating FDOT prequalification in the applicable work classes in the submittal bid. The awarded contractor must be registered and licensed in the State of Florida to perform the duties necessary to complete the various services for this project. Must be submitted in writing via the ProcureNow Question/Answer Tab via the City's eProcurement portal, on or before, Friday, November 4, 2022 by 5:00 pm. All Correspondence must reference the Bid Number. When asking questions, please be sure to enter each question separately. Providing all labor, materials, storage facilities, equipment and incidentals necessary for roadway reconstruction, bridge replacement, milling and resurfacing, maintenance of traffic, and signing and pavement markings for the 5th Street Bridge in Volusia County. The work entails full replacement of the 5th Street Bridge including demolition of the existing bridge structure, constructing a temporary bridge prior to the demolition process as there is only one access to Commodore Island, and constructing a new longer multi-span bridge within the same footprint as the existing bridge. Demolition entails complete removal and proper disposal of the existing bridge deck, superstructure and substructure, end bents, pilings, abutments, revetment, rip rap embankments, guardrail system, roadway approaches and in-situ soils to extend the waterway opening as shown on the plans. The temporary bridge in form of a two lane Acrow type structure can be provided by the Contractor or leased from the State via agreement through the City, whichever option is determined to benefit the City. Reference the alternate for the lease option from the State Aluminum Structures Shop. The existing single span +-35' bridge will be replaced with a three (3) span structure totaling 160' including a 5' pedestrian walkway along the north side of the proposed bridge. The existing structure has no sidewalk. The Contractor will be responsible for replacement of the roadway approaches, ongoing maintenance of traffic in accordance with MUTCD & ATSSA guidelines, material testing, drainage improvements, maintaining stormwater pollution prevention devices, pavement markings & signage and site restoration. The Contractor will also be required to provide administrative services including reports with ongoing project schedule updates, adherence to all agency permit conditions included within the bid documents, secure and coordinate all laydown/storage areas needed to facilitate deliveries and extended storage of materials, setup utility accounts and pay for temporary water/electric services, obtain and pay for City permits, either perform or retain a Geotechnical Engineer for vibration monitoring, submit all required FDOT documents within the specifications, submit and attain shop drawing approval for all bridge components, abide by OSHA requirements for bridge workers, and provide close out documentation such as pile driving logs, record drawings and certification of completion
